Charitable Solicitations Disclosure Statements

Some states require charitable solicitation disclosure statements to be included on all written materials mailed to potential contributors in their states.  The following is a list of disclosure statements that must be printed on all charitable solicitations mailed in to the residents of these states.  This information is not intended as legal counsel and does not constitute legal advice. 

 

(Name of Charity) is a charitable organization exempt from taxation under section 501(c)(3) of the Internal Revenue Code, and as such, we are required to include certain disclosure language on all printed solicitations.

Florida: A COPY OF THE OFFICIAL REGISTRATION AND FINANCIAL INFORMATION MAY BE OBTAINED FROM THE DIVISION OF CONSUMER SERVICES BY CALLING TOLL-FREE (800-435-7352) WITHIN THE STATE OR VISITING WWW.FDACS.GOV.  REGISTRATION DOES IMPLY ENDORSEMENT, APPROVAL, OR RECOMMENDATION BY THE STATE.

Georgia: A full and fair description of the programs and activities of (Name of Charity) and its financial statement are available upon request at the address indicated above. 

Illinois: Contracts and reports regarding (Name of Charity) are on file with the Illinois Attorney General. 

Kansas: In Kansas our license number is (Charity's registration number). 

Maryland: A copy of the current financial statement of [name of charity] is available by writing [address where requests should be directed] or by calling (XXX) XXX-XXXX. Documents and information submitted under the Maryland Solicitations Act are also available, for the cost of postage and copies, from the Maryland Secretary of State, State House, Annapolis MD 21401, (410) 974-5534.

Mississippi: The official registration and financial information of (insert legal name of the charity as registered with the Secretary of State) may be obtained from the Mississippi Secretary of State's office by calling 1-888-236-6167.  Registration by the Secretary of State does not imply endorsement by the Secretary of State.

Nevada: The state of incorporation of (Name of Charity) is _________.  

New Jersey: INFORMATION FILED WITH THE ATTORNEY GENERAL CONCERNING THIS CHARITABLE SOLICITATION AND THE PERCENTAGE OF CONTRIBUTIONS RECEIVED BY THE CHARITY DURING THE LAST REPORTING PERIOD THAT WERE DEDICATED TO THE CHARITABLE PURPOSE MAY BE OBTAINED FROM THE ATTORNEY GENERAL OF THE STATE OF NEW JERSEY BY CALLING 973-504-6215 AND IS AVAILABLE ON THE INTERNET AT http://www.state.nj.us/lps/ca/charfrm.htm. REGISTRATION WITH THE ATTORNEY GENERAL DOES NOT IMPLY ENDORSEMENT. 

New York: Upon request, from the Attorney General Charities Bureau, 120 Broadway, New York, NY 10271.  

North Carolina: Financial information about this organization and a copy of its license are available from the State Solicitation Licensing Branch at 1-888-830-4989 within (within North Carolina) or (919) 807-2214 (outside North Carolina).  The license is not an endorsement by the State.

Pennsylvania: The official registration and financial information of (Name of Charity) may be obtained from the Pennsylvania Department of State by calling toll-free, within Pennsylvania, 1-800-732-0999. 

Virginia: Financial statements are available from the State Office of Consumer Affairs, PO. Box 1163, Richmond, VA 23218. 

Washington: The (name of organization) located in (city) is registered with Washington State’s Charities Program as required by law; financial and additional information is available by calling 360-725-0377 or visiting https://ccfs.sos.wa.gov/#/ on file at Charities Division, Office of the Secretary of State, PO Box 40234, Olympia, WA 98504.

West Virginia: West Virginia residents may obtain a summary of the registration and financial documents from the Secretary of State, State Capitol, Charleston, West Virginia 25305.
